4, 5, 5, The mirror like surfaces at 10 o'clock and 4-6 o'clock appear to be very lightly mottled. I'm not sure if this is a planchet defect. But the "shiny spots" are irregular and when at the correct light angle, interefer with the "watery surface." Thus a 4 on the grading for the planchet. Light splotchy toning. Perhaps it needs to be dipped. The photo shows a smear at the bottom right, about five o'clock. This is a reflection, not on the coin or on the holder.
